What is a midline discrepancy?

The dental midline is an imaginary vertical line that runs between the two central incisors of the upper and lower jaws. Ideally, this line should align both between the jaws and with the center of the face.
When this line does not match, we talk about a midline discrepancy (also called midline shift). A well-known example is the actor Tom Cruise.

Why is it important to treat?
The most common reason patients seek treatment is aesthetic. The midline is located at the center of the face, within the so-called aesthetic triangle (eyes–nose–mouth). This is the facial area that people naturally focus on during conversation. Beyond appearance, a shifted midline may also cause functional problems, such as uneven tooth wear, difficulty chewing, or temporomandibular joint (TMJ) pain.
Most common causes:
Premature loss of baby teeth – can lead to drifting of neighboring teeth and midline asymmetry.
Missing teeth or crowding – teeth tend to move toward the empty space.
Smaller-than-average permanent teeth – size discrepancies can shift the midline.
Impacted teeth – teeth that remain trapped in the bone may cause asymmetry in the dental arch.
Anatomical or hereditary factors – such as jaw size asymmetry.
Oral habits – thumb sucking, tongue thrusting, or prolonged infantile swallowing patterns that disturb tooth position and midline alignment.

Treatment options:
Midline discrepancies are usually treated with braces or aligners, often combined with additional techniques:
- Elastics – often worn diagonally to correct midline mismatches, especially in mild to moderate cases.
- Interproximal reduction (IPR) – involves removing a very small amount of enamel between teeth to create space and improve bite without extractions.
- Complex cases – may require tooth extractions (e.g., if one side already has a missing tooth) or even orthognathic surgery in severe cases.
Importance of early diagnosis
The earlier a problem is detected, the more effective and less invasive the treatment can be. In children, minor and shorter interventions often achieve excellent and stable results. That’s why the first orthodontic check-up is recommended around the age of 6, when potential problems can be addressed before they become more complicated.
